Apply to NDSM

Registration forms can be found in the front offices and on this site starting in January. Please return forms as quickly as possible to secure your students seat in their grade. After parents register their child with the school, they can apply for the Milwaukee Parental Choice Program (MPCP) or Wisconsin Parental Choice Program (WPCP) on DPI’s website. A complete application for Notre Dame includes:

• Notre Dame Registration Packet
• Completed online application for MPCP/WPCP (see Choice instructions below)

Apply to Choice

The 2023-2024 Milwaukee School Choice Program is open again until March 20th and Wisconsin Choice is open until April 20th.  All NDSM students must apply for Choice as part of enrollment.  Choice applications are submitted through a Department of Public Instruction (DPI) website:

You’ll need the below items to complete the Choice application:

Important Notes

  • You must send in a copy of your residency document (for all families) and also your income document (for students new to Choice) in order to complete your application with NDSM
  • Your application cannot be verified without these important documents
  • Please drop off all documents at a lockbox located inside the entrance way at either the BTC campus (1420 W. Scott St., Milwaukee, WI 53204) or the MCC campus (1425 S. 26th St., Milwaukee, WI 53204)
  • Check Class Dojo for more information

Lottery

If more completed applications are received than spots available for any given grade level, a random lottery is conducted to determine which child is admitted to the school. If a sibling is currently enrolled at the school, subsequent siblings are granted admittance in their grade, as room permits. A letter will be sent to all applicants regarding their application status and their number on the wait list, if applicable.

Health and Wellness

NDSM provides nutritious breakfasts and lunches to our students at no cost to our families. This is due to our being a Community Eligibility Provision (CEP) school. To learn more please read our Health and Wellness Policy.

Parental Involvement at NDSM

Parents are encouraged to volunteer at least 20 hours per year. Attendance at the Safe Environment training and a background check are required by the Archdiocese of Milwaukee for all volunteers. This training is presented in both Spanish and English. Dates and times for this training can be found here. It is only required to complete this training one time and is transferable to all other Archdiocese of Milwaukee schools.

We strongly encourage our families to attend school functions, students activities and parent meetings. Family participation in such events makes NDSM a better school and creates a more informed, caring, and culturally rich community.
